Nominate a Caregiver
Caregivers play a vital role in providing compassionate support and care to patients and their families in hospice. This page allows you to recognize and appreciate these dedicated individuals by nominating them for their exceptional service.
Eligibility
We know it takes a special person to work or volunteer in hospice. Any hospice employee or volunteer who provided care or support directly to a patient or their family is eligible to be nominated for the award.
- Executive directors and administrators
- Nurses and nurse aides
- Social workers
- Bereavement staff and chaplains
- Volunteer coordinators and volunteers
- Therapy (music, art, etc.) staff
- Office support staff
Each nominee will receive a personally delivered small gift as a token of appreciation for the wonderful care they provide.
How to Nominate
You can nominate a hospice caregiver or co-worker who you believe exemplifies what it means to be a hospice caregiver. If you would like to nominate someone, please complete the nomination form below.
The selection committee looks for information about specific acts of compassion and caring. When nominating an individual for the award, be sure to include specific details about the way the individual works with patients and their families, or tell a specific story about something amazing this caregiver did while caring for a patient or their family.
